CDM/PCN Updates- Staff View
The Original PCN page has not been restructured and expanded to handle CDM Components. Functionality remains the same , the view and use has been changed to accommodate future developments.
Patient Profile HMC CDM Tab
New section: Reminder Summary
A new section has been added to when the next Assessment is recommended to be done according to CDM guidelines.
This section will display Last assessment done date and what is expected in the upcoming next 6 months.
New Section: Patient condition selection method
New section : Condition cards
Each condition will have their own Card with the related assessments in consultation flow and the division allows for easy separation of each condition.
Each complication will have related SNOMED Codes.
Post Consult HMC CDM Tab
This tab will be visible in Post consult as well, with Vitals and assessments for each condition
PCN Overview Page
The PCN page will also display these changes , but data entry will remain the same , if the data is auto loaded by Innoquest this will still remain.
