Creation of Lab Request

When patient is coming to the clinic for a lab test / package / profile to be done.

Step 1: After adding the patient to CMS on patient registry page, select the Actions button and select “Print Lab Form”.

a.      A request pop up will appear and
staff will have to select the following before adding the lab tests:

      • Lab
        – Select option as “INNOVATIVE DIAGNOSTICS PTE LTD”
      • Doctor

 

Step 2: Staff can proceed to add the lab tests – only INNOVATIVE lab codes will be reflected (based on the lab selected).

    i. Add rows according to how many tests are to be added.

    ii. If you are adding a package / profile – just select the package / profile code will do.

   iii. The remarks for ad-hoc scenarios can now be indicated under the remarks column in free text.

Note:
In the event, you do not have the Innovative lab code in CMS, you may also type the test name in free text according to the lab / profile code.


Step 3:
Proceed to “Print Request Form” and “Print Lab Label”.


Step 4:
Staff may continue with the normal process flow and pass these printed items to doctor.
