CDM/PCN Updates- Doctor View
The Original PCN page has not been restructured and expanded to handle CDM Components. Functionality remains the same , the view and use has been changed to accommodate future developments.
Patient Profile HMC CDM Tab
A new tab will display the CDM information, when viewing patient without registration you can view/update their PCN/CDM condition.
New section: Reminder Summary
A new section has been added to when the next Assessment is recommended to be done according to CDM guidelines.
This section will display Last assessment done date and what is expected in the upcoming next 6 months.

New Section: Patient condition selection method
Previously condition was selected via radio buttons with all conditions displayed, now it’s a drop down with search feature.
search by typing in
New section : Condition cards
Each condition will have their own Card with the related assessments in consultation flow and the division allows for easy separation of each condition.
Each complication will have related SNOMED Codes.
You can modify and select related SNOMED code when needed.
Consultation Flow HMC CDM Tab
- CDM tab will always be visible.
- It is only mandatory if Patient is PCN and visit is Chronic visit.
It looks same as above mentioned CDM conditions but you will also see Vitals and Assessments per conditions
common Assessments need to only be filled in one card and it will auto fill in other cards.
